Well-Child Checks/Physicals: Required for grades K and encouraged yearly for following school years. Required for anyone who participates in middle school athletics. Medical approval is required for grades 5 and 7 who wish to play sports.
Annual Health Update and Medication Permission Form: All parents please fill this out and return.
Immunizations: Your child must be up to date on immunizations before the new law takes into effect on September 1st, 2021. No child will be permitted to attend school until required vaccines have been administered.
Does your child need an action plan? Examples are severe food allergies which requires epinephrine and asthma which requires inhalers. Both action plans need to be signed off by their healthcare provider yearly. Asthma and Epi-Pens also require a signature from their healthcare provider on the “AOS 91 Medication Administration Form”.
Does your child need to take prescribed medication during school hours? Parent/ Guardian and their healthcare provider need to sign the “AOS 91 Medication Administration Form”.

Kindergarten through Second Grade at Seal Harbor Beach
When there is a change in your child's dismissal routine ( getting picked up instead of riding the bus, different adult picking up your child, leaving early for an appointment, etc), please let the office know by calling 244-5502. This will help eliminate confusion, anxiety and delays at dismissal time. When dropping off or picking up your child at irregular times, please call the office to let them know. These are really important steps that must be followed for the safety of all of our students.

After a year off, The MDI Girls Basketball Program is proud to announce we will be running our annual basketball camp again this year. Our camp will be from June 14-18 from 3:30-6:00 for all girls entering the 6th-9th grades. As we get closer to camp we may have some changes in restriction but as of right now MASKS will be Mandatory inside the gym. All the information is attached below, I hope to see you all there.

2021 MDI Girls Summer Basketball Camp
June 14-18 3:30pm-6pm Girls Entering 6th - 9th Grade
Camp Features: Skill stations, Full court drills, Tournaments (5-on-5, 3-on-3, 1-on-1), Daily Competitions (Hot Shot, Free throws, 3-point, Skills challenges) The clinic will focus on fundamentals and defense. The emphasis will be on encouraging all players to develop a greater understanding of each aspect of the game of basketball. The goal is to develop players who are serious about basketball and to combine this knowledge with the hard work necessary to improve skill level. This camp is for everyone from beginners, to advanced players who want to become a better basketball player.
Camp Counselors: Current and former MDI Girls basketball players and coaches
What to bring: Basketball shoes/sneakers, t-shirt, athletic shorts, water, snacks.
I hereby authorize the staff at the MDI Girls Summer Basketball Camp to use their best judgment in any emergency requiring the use of local emergency facilities. I also certify that my child is physically able to participate in all activities and I assume all risks associated with participating in the program. I also fully understand that the MDI Girls Summer Basketball Camp does not provide medical insurance. Registration requires that a parent/guardian sign below, agreeing that in the case of an accident involving your child, he/she releases the camp, sponsors, counselors, and directors from any and all liability.

Parents -- Self Care is important! Remember the announcements on planes to put your own oxygen mask on before helping others? The stresses of the pandemic coupled with the stresses of parenting can make it so you forget to take care of YOU. Check out these Self Care Suggestions for Parents. You may want to review the list and select one or two to incorporate into your daily routines. You could add a self care practice to things you already do (e.g., with your cup of coffee). If you talk with someone else and share what you plan to do, you will be more likely to do it. Then you can check in about how it is going, review the list of suggestions, get and give encouragement, etc.
Becoming active and involved in your child’s education, builds healthy children and a healthy community. A great way to do so, is by joining the Pemetic PTO.
