Call for Working Group members:
Consistent Definitions for Financial and Administrative Data

At the CCSN Leadership Table May meeting, it was agreed that greater consistency in financial reporting would be added to the 2019-2020 Action Plan. We would like to invite interested members to join a time-limited working group to review and develop consistent definitions for financial and administrative data.

The working group will come together through online meetings to discuss the current understanding of reporting codes and make recommendations for consistent definitions. It is expected that the group will meet on a monthly basis until November 2019.

Caregiver Wellbeing Index – Supportive Pathways for Caregivers resource available in French

In February of 2019, select agencies across the region implemented the Caregiver Wellbeing Index (CWBI) as a way to formally screen Caregivers for distress related symptoms, and based on their score to connect them with appropriate supports and resources. As part of this pilot, the Supportive Pathways for Caregivers resource was created as a way to help front line staff navigate caregivers to supports based on their CWBI score. You can now access the Supportive Pathways resource in French.

Improving access to Stroke based community resources – Partnering with the Champlain Regional Stroke Network on eReferral 

CCSN has been working with the Champlain Regional Stroke Network (CRSN) to identify opportunities for collaboration and system improvement and improve transitions post-stroke to appropriate community-based supports and resources. On the Community Support Service e-referral portal, there is now a Stroke button available under the Conditions heading  to provide easy access and navigation to stroke based community services. Over the coming months, the list of available stroke-related community services through eReferral will continue to grow. 
As a second stage to this work, the partnership will be providing education and outreach to hospital and rehab staff and discharge teams across our region, to help facilitate easy access to key resources in the community through eReferral.
