SCHOOL HEALTH READINESS ASSESSMENT
*Notice: We understand at this time schools are closed due to COVID-19.  Our School Health Readiness Assessment will be going live as planned on April 1, 2020.  Please take this time to consider what three Whole School, Whole Community, Whole Child (WSCC) Model components will be top priorities for your school in the 2020-2021 school year.  We will not have in-person IAK Planning Meetings until we are cleared to do so.  We would like to offer IAK Planning Meetings via phone conference.  This will allow us to get programming scheduled for the 2020-2021 school year.  Once you have completed the assessment for your school and we have received it, our team will reach out to schedule an IAK Planning Meeting conference call.  We would like to remind you that we do work over the summer as well and will be available to meet.  We are thinking of all of you, stay safe!

It’s All About Kids (IAK) is proud to partner with any elementary school in Tulsa County!  In an effort to adequately meet the needs of our partnering schools and provide data to the Oklahoma State Department of Health we have developed a process.  IAK requires Tulsa County elementary schools interested in partnering with us to complete an IAK School Readiness Assessment along with having a district Memorandum of Understanding (MOU).  The assessment allows us to gain an understanding of your school's climate and tailor our services accordingly.  The IAK Assessment for the 2020-2021 school year will be accepted from April 1, 2020 - September 1, 2020.  Assessments received after the September 1, 2020 deadline will not be accepted.   The information needed to complete the assessment will come from the 2019-2020 school year.  As a reminder the official completed assessment for your school must be submitted electronically.  If you are unable to answer a question simply put N/A.

We recommend completing the assessment with the following school personnel: principal, assistant principal, grade level team leads, PE teacher, nurse/health assistant, counselor, and community coordinator/parent liaison.  Please have all appropriate information gathered before beginning the assessment.  Once the assessment is started you cannot exit without losing the entered information and having to start over.  A PDF version of the assessment is available for your team to review and gather information before completing the online assessment.  Our assessment aligns with the CDC’s Whole School, Whole Community, Whole Child (WSCC) Model.  You will be asked to select your top 3 WSCC component areas and only complete the assessment pertaining to the components chosen.  Please review the WSCC Model Components & IAK Topics list to be sure you select the appropriate component areas for your school.  We ask that your school choose topics within the 3 WSCC components you select.

Once your assessment has been submitted the IAK team member assigned to your district will contact you to schedule an IAK Planning Meeting for the 2020-2021 school year.  We prefer to schedule all planning meetings between April 1, 2020 and September 1, 2020 in an effort to be prepared and ready when the school year begins.  All IAK Planning Meetings must be complete by October 1, 2020 in order for IAK to partner with your school this year To complete the assessment for your school, please visit our website http://www.tulsaplay.org/IAKSchool

Remember to follow us on our social media, @IAKTulsa, for school health and program information.  We look forward to partnering with you for the 2020-2021 school year!
